Community Services (Home Team)  – Dunedin based

 

  • Part time position (32hours per week)
  • Monday to Friday

This is a fantastic opportunity for a Registered Nurse Practitioner or Registered Nurse who is on the Nurse Practitioner pathway.  You will be expected to have completed the pathway in 2 years while working as part of our Home Team and Older Persons Health Team.

 

About the role/ The team

The Home Team is an interprofessional team currently consisting of Occupational Therapists, Physiotherapists, Registered Nurses and Rehabilitation Assistants.  Their interprofessional practice is underpinned by the Calderdale Framework to support skill sharing among the professions.  The Home Team provides a hospital avoidance and supported discharge service including a community rapid response service.  Whilst they cater for all ages above 16 years, their work is predominantly with older persons or those who are close in age and interest.

 

The primary purpose of this position is to:

  • Promote partnership and integration across primary, community and secondary services for the Home Team and OPH.  The Home Team provides a rapid response service to the community alongside a supported discharge service.
  • Work across services to increase early, proactive assessment and intervention of complex clinical issues in the community and to build capability within the Home Team to manage patients with more complex conditions safely in the community.
  • Provide comprehensive assessment and treatment in the community alongside the consultant geriatricians, OPH Nurse Practitioner, Home Team and other involved practitioners.  Provide consultancy and clinical leadership within the Home team.
  • Provide clinical nursing expertise and leadership related to the management of people with complex health issues, particularly the older person, to teams in primary and secondary care.
  • Develop and influences education, health policies, clinical standards/practice and service development at a local level with a quality improvement focus.
  • Contribute and support the development of a frailty pathway for SDHB.
  • Provide advanced nursing care for people with dementia, delirium, and those older person’s presenting with significant challenging behaviour, through the development of a collaborative/ interprofessional model of care with the interdisciplinary team.

 

About you

The successful candidate will have extensive experience in Older Persons Health or Primary Care, and acute care, exceptional communication skills, and the ability to form effective working relationships across multiple teams and services within the wider health sector. 

 

To succeed in this exciting new role, you will also need to:

 

  • Be a Registered Nurse Practitioner with a valid Annual Practicing Certificate or a Registered Nurse on the Nurse Practitioner Pathway with an expected completion date of 2 years
  • Minimum of 5 years’ experience in Older Persons Health or Primary Care and ideally acute care
  • Have the ability to successfully engage stakeholders in a positive manner
  • Have demonstrated knowledge and application of NZ Health strategies
  • Be a fabulous communicator with an engaging and personable style
